This is a legally binding liability release, waiver, discharge and covenant not to sue (the “release”), made by me, the undersigned, to Gonzalez Jiu Jitsu DBA Texas Grappling Academy (the “gym”) to the instructors of the gym (the “instructors”) and the members of the gym (the “members”). The undersigned participant using Gonzalez Jiu Jitsu DBA Texas Grappling Academy equipment or while on the property, voluntarily makes and grants this waiver and assumptions of risk in favor of Gonzalez Jiu Jitsu DBA Texas Grappling Academy, its owners professors, instructors, and respective entities referred to as service providers herein as partial consideration for the opportunity to use the facilities, equipment, material and/or other assets of the service providers; and/or to receive assistance, training, guidance, tutelage and/or instructions from the personnel of the service providers; and/or engage in the activities, events, sports, festivities and/or gathering sponsored by the service providers; I do hereby waive and release any and all claims whether in contract or of personal injury, bodily liability injuries, property damage, damages, losses and/or death that may arise from my aforementioned use or receipt, including but not limited to negligence, mistake, or failure to supervise. I understand and recognize that there are certain risks, dangers and perils connected with such use/or receipt, which I hereby acknowledge have been fully explained to me and which I fully understand, and which nevertheless accept, assume and undertake after inquiry and investigation of extent, duration and completeness wholly satisfactory and acceptance to me.
I further agree to use my best judgement in undertaking these activities, use and/or receipt and to faithfully adhere to all safety instructions and recommendations, whether oral or written. I hereby certify that I am a competent adult/parent/legal guardian assuming these risks of my own free will, being under no compulsion or duress. I Fully recognize that there are dangers and risks to which I may be exposed by joining the gym and participating in Brazilian Jiu-Jitsu, Mixed Martial Arts, Submission Grappling, Boxing, group workouts and kickboxing which involves strenuous physical activities and extensive physical contact with other people and a mat, included but not limited to joints locks, throws, chokes, strikes ground fighting and other similar activities. Further, you represent that you/your child are in good physical condition and have no medical reason for impairment that might prevent you from your intended use of Gonzalez Jiu Jitsu DBA Texas Grappling Academy. As such you acknowledge that the service providers did not give you medical advice before you joined, and cannot give you after you join. Relating to your physical condition and the ability to use the facility. If you have any health or medical concerns now or after you join, discuss them with your doctor before using the facility.
I recognize that this release means I am giving up, among other things, right to sue Gonzalez Jiu Jitsu dba Texas Grappling Academy, its agents, the instructors, professors, providers, and the members for injuries, damages, or losses I may incur while participating in any activities. I also understand that this release binds my heirs, executors, administrators, and assigns, as well as me.
I have read the entire release; I fully understand it and agree to be legally bound by it. This waiver and assumption of risks is effective from this day forward inclusive and may not be revoked, altered, amended, rescinded or voided without the express prior written consent of the service providers.

BRAZILIAN TOP TEAM CODE OF CONDUCT
Traditionally, the study of the martial arts consists of both the practice of skills and the adherence to a code of conduct, or ethics. Brazilian Top Team dedicates entirely to the formation of new talents in martial arts. The code of student conduct is established to foster and protect the core missions of the academy, to foster the formation and civic development of BTT Somerset students in a safe and secure learning environment, and to protect the people, properties and processes that support the academy and its missions. The core missions of the BTT Somerset are formation, teaching, learning, and good service.
CODE OF CONDUCT AND DOJO RULES THAT EVERY STUDENT MUST FOLLOW
- All students must wear clean BTT Gi's for Gi classes and BTT rash guards for No-Gi classes
- All GI’s must have BTT patches to train, drills or compete
- All students must be on the mat minutes before scheduled class time
- All students will bow when entering or leaving the mat
- All students must always bow individually to every black belt present in the mat
- It is impolite to ask a higher ranking belt student to train. He/she will ask you
- All students, including non-teaching black belts will bow to the instructor, (whether or not the instructor is a black belt) when entering or leaving the mat
- All students must have a t-shirt or rash guard for any grappling training and lifting room to work out
- All students must respect the training partner and bow before and after training
- All students must change the clothes on the restroom or change room
- Absolutely no foul language inside the school
- During class, when instructor is demonstrating the techniques, every student must sit in seize. No talking during instructions
- Keep finger and toe nails clipped and clean at all times
- Women Gi and No-Gi must be properly dressed in BTT rash-guards and grappling wear pants
- Remove all metals, jewelry, piercings, necklaces, rings etc. prior to train or drills
- You can use any mat area at gym when there is no class going on, but if a class is in progress you cannot (BJJ mat, cage, boxing mat, conditioning area, etc.)
- Do Not ask to use the mat if you are not going to make to the class
- Do not ask to share the mat during any classes
- No student may leave the mat for any reason without express permission of instructor
- No cell phones, pagers, beepers or other electronic devices will be allowed in the mat area, without express permission from the instructor
- Absolutely no shoes, gum, food or drinks on the mats
- Priority of the mats are for the classes 23. Always remember when you are in public, you are a representative of Brazilian Top Team and its good name, and don't forget to have fun and never tired
